Who we are

Aprovo provides care between visits for Medicare patients. Our AI calls patients on behalf of the medical practices and provider organizations we work with, and hands anyone who needs a clinician to theirs. This policy explains what we do with information collected through our website at aprovohealth.com.

What this policy covers, and what it does not

This policy covers our website and the people who visit it or email us. It is about business contact and website information, not medical information.

It does not cover patient or clinical information handled in our service. If you are a patient of a practice that works with us, we handle your health information on that organization's behalf, under our agreement with them, including any business associate agreement. Your rights in that information run through that organization and its own notice of privacy practices, not through this policy. If you are not sure which applies to you, email us and ask.

What we collect

Information you send us. The website has no forms, no accounts and no payments. The only way to give us information through it is to email the address published on it. When you do, we receive your email address, your name and signature if you include them, and whatever you write. We keep that correspondence so we can reply and continue the conversation.

Information collected automatically. When you load a page, we and the providers described below receive technical information about the visit. That includes your IP address, your device type, operating system, browser, language, country and time zone, the pages you viewed, and the page or link that referred you.

Inferences about the organization you visit from. Our website visitor provider uses your IP address and device identifiers to infer or enrich information about the visit, such as the company, industry and location associated with it. In practice this means we may learn which organization appears to be looking at our site, and that our provider may match the visit to business contact records it already holds. We use this to understand which kinds of organizations are interested in us and to follow up with them at work.

Cookies and similar technologies

Our website visitor provider sets a first-party cookie in your browser and uses cookies and browser local storage to recognize your browser across visits. We do not use cookies for advertising, and we do not run advertising on this site.

You can block or delete cookies and clear local storage in your browser settings. The site works without them. Browsers handle Do Not Track inconsistently and we do not respond to it. If you would rather we did not hold website information about your visit, email us and we will remove it.

Why we use it

We use website information for a short list of things:

To reply to you and keep a record of the conversation. To understand which organizations are interested in what we do, and to reach out to people at them about our service. To see which pages are read and how people move through the site, so we can write a better one. To keep the site working, and to investigate abuse, errors and security problems.

We do not use website information to make decisions about anyone's medical care, and we do not combine it with patient information.

Who receives it

Service providers. We use third parties to run this site and our business, and they receive information for the purposes described above and nothing else. They are an analytics and website visitor identification provider, which operates the tracking and the cookie described above; a font and content delivery provider, which serves the typeface your browser loads and receives your IP address and browser information as part of that request; and our hosting, email and business software providers, which handle information on our behalf so we can serve the site and read our mail.

Some of these providers handle information under their own privacy policies rather than only on our instructions. We will tell you which providers we use, by name, if you email and ask.

We also disclose information if the law requires it, to protect our rights or someone's safety, or to a buyer or successor if the business is sold, in which case this policy continues to apply to the information transferred until it is replaced with notice to you.

We do not sell your personal information, and we do not share it for cross-context behavioral advertising or targeted advertising.

How long we keep it

We keep email correspondence for as long as we are in contact and for a reasonable period afterward, so we have a record of what was said. We keep website visit information for as long as it is useful for the purposes above, and delete or aggregate it after that. If you ask us to delete something sooner, we will unless we are required to keep it.

How we protect it

We limit access to the people who need it, use providers that encrypt information in transit and at rest, and keep the amount we hold small on purpose. The website collects no medical information and no payment information, so there is none of either to lose here.

No method of transmission or storage is completely secure, and we do not promise otherwise. Please do not send medical details or other sensitive information to us by email.

Your choices

Email founders@aprovohealth.com and we will, for information we hold about you: tell you what it is, correct it, delete it, send you a copy, or stop using it to contact you. Ask for any of these in plain words. You do not need to cite a statute, and we do not charge for it or treat you differently afterward.

We may need to confirm you are who you say you are before we act, and we will not use what you send us for identification for anything else.

California privacy rights

If you live in California, the California Consumer Privacy Act as amended by the CPRA gives you specific rights. This section explains how they apply here.

Categories of personal information we collect. Identifiers, including your name, email address and IP address. Internet or other electronic network activity information, including the pages you view and how you arrived. Professional or employment-related information, where our visitor provider infers the organization a visit came from. Inferences drawn from the above about the organization you are visiting from and its likely interest in our service. We collect these from you directly when you email us, automatically when you use the site, and from our website visitor provider.

Sensitive personal information. We do not collect it through this website, and we do not use or disclose any for purposes that would require us to offer you a right to limit that use.

Sale and sharing. We have not sold personal information and have not shared it for cross-context behavioral advertising in the preceding twelve months. We disclose it to service providers for the business purposes described above.

Your rights. You may request to know what we collect and disclose, request a copy, request correction, and request deletion. You may opt out of any sale or sharing, though as stated we do neither. We will not deny you service, charge you a different price or give you a lower quality of service for exercising any of these. You may use an authorized agent, and we may ask for proof of their authority. Email the address above to exercise any right, and we will respond within the time the law allows.

Children

This site is meant for the people who run medical practices and the firms that invest in them. It is not directed to children, and we do not knowingly collect personal information from anyone under 16 through it. If you believe a child has sent us information, email us and we will delete it.

Where information is handled

We are based in the United States and the providers we use handle information in the United States. If you visit from another country, you are sending information to be handled there.

Changes to this policy

When this policy changes we will update it here and change the date at the top. If a change matters to you, for example if we begin collecting something new or using it for a new purpose, we will say so clearly rather than quietly editing the text.
